What are the responsibilities and job description for the Registered Nurse Adult Day Care position at Solace Gardens LLC dba SarahCare at Riverdale Pla?
The Registered Nurse Adult Day Care will provide clinical oversight and nursing services to participants enrolled in SarahCare at Riverdale Place Adult Day Health Center. This role ensures high-quality, person-centered care by conducting assessments, developing and overseeing individualized plans of care, coordinating services, and maintaining compliance with SarahCare standards, all adult day health care requirements, state and federal guidelines, and VA and Medicaid requirements. Duties are:
- Oversight supervision of Licensed Practical Nurse and Direct Care Assistants.
- Administration of medications as ordered by the participant’s physician and appropriate disposal of expired/discontinued medications.
- Documentation of any necessary medical observations and daily record keeping.
- Update participant’s medication orders, treatments, and medical questionnaires as required.
- Communicate with the Participant’s responsible party and physicians as required.
- Documentation of Administrative Reports as needed.
- Monitor participant’s vital signs as required.
- Conduct staff in-service/education on relevant topics as required.
- Participation in assessment and Person-Centered Plan of Care conferences as required.
- Assists participants with activities of daily living, personal care, and in the participation of activities.
- Participate in staff meetings, in-service training, group discussions, and other relevant training.
- Requests supplies and items needed for nursing, through the Program Director.
- Ability to safely lift and transfer.
- Responsible for environmental safety and sanitation.
- Follow Standard Precautions at all times.
- Comply with all HIPAA and confidentiality requirements.
- Administration of emergency CPR/First Aid as needed.
