Sofavi Home Health LLC is hiring part time nurses for a pediatric client.
- Friday daytime is critical. Other shifts available if desired.
SKILLED CONTRACT NURSE 1:1 FOR PEDIATRIC PT. START DATE IMMEDIATE, with PAID TRAINING.

EXPERIENCE IN PEDIATRICS AND TRACH/VENT PREFERRED.
THIS IS A CONTRACT HOURLY POSITION FOR FRIDAY DAYTIME; HOURS FLEXIBLE BUT 7AM-5PM PREFERRED. ADDITIONAL WEEKEND HOURS AVAILABLE IF CANDIDATE DESIRES. BIANNUAL BONUS STRUCTURE OFFERED TO CONTRACTED NURSE AFTER 6 MONTHS and 12 MONTHS.
Provide skilled nursing care to a trach/vented patient in the home setting.
Provide and coordinate nursing care for the patient in collaboration with the Health Care team. The contracted Nurse is an integral member of the Health Care team, overseeing and participating in patient assessment and the formulation and implementation of the care plan.
Essential Responsibilities:
- Using the nursing process, provides ongoing assessment of the patient. Follows a written plan of care for patient care needs. Assists patient and family in advancing and developing health care goals which are part of the individualized plan of care. Provides nursing interventions safely and competently in the home or school setting. Provides the health care team, patient and family with clear communication about all interventions. Provides ongoing evaluation of the patient's response to care. Anticipates and recognizes changes in the patient's status and need for nursing care. Modifies the individual plan of care in collaboration with the health care team.
- Demonstrates effective communication skills.
- Collaborates, communicates and coordinates with other health care team members and family in planning and implementing care.
- Cooperates and communicates effectively with other team members in order to accomplish the workload.
- Identifies the patient's educational needs and progress as part of the nursing assessment.
- Encourages patient compliance with therapeutic and medical regimens.
- Assesses patient school-related risk factors.
- Integrates health promotion and preventive aspects of care with the ongoing assessment of nursing care needs.
- Works with patient and family to develop a plan to improve general health status.
- Implements physical and psychological preventive measures.
- Provides a safe environment for the patient even in uncontrolled settings.
- Adheres to infection control standards and guidelines, including the guidelines for reporting communicable diseases.
- Identifies own learning needs and participates in continued education programs to meet those needs.
- Participates in student education activities.
- Assures quality of care through development, implementation and evaluation of professional nursing standards and participation in multidisciplinary quality assurance activities.
- Maintains a high standard of professional work habits.
- Provide care coordination/management for chronic medically complex or fragile patients.
- Incorporate prevention: address patient care need gaps; act as patient advocate. Assess, anticipate & accommodate for age-specific needs, accommodate patient disabilities & limitation, access resources for school therapy services.
- Work collaboratively to develop, coordinate, & implement a plan of care that is patient & outcome focuses.

Education
- Associates degree in Nursing from an accredited program.
License, Certification, Registration
- Current Washington State LPN or RN License by date of hire/transfer.

- Communication, problem-solving, attention to detail, leadership and independent decision-making skills.
Preferred Qualifications:
- Respiratory care nursing experience
- Primary nursing or case management experience.
- Bachelor of science in Nursing (BSN).
