Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine individualized care needs.
- Develop and implement tailored care plans that align with physician orders and best practices for home health services.
- Manage patient care through skilled nursing interventions such as wound management, medication administration, and infection control procedures.
- Coordinate discharge planning and post-acute care strategies to facilitate smooth transitions from hospital or facility to home.
- Utilize electronic health records (EHR/EHR) systems for accurate documentation of patient records, medical histories, and clinical notes while ensuring compliance with HIPAA regulations.
- Collaborate effectively with healthcare professionals—including physicians, therapists, social workers—to optimize patient outcomes through case management and interdisciplinary communication.
- Educate patients and their families on health management, medication adherence, infection prevention, and lifestyle modifications to promote long-term wellness.
Requirements
- Valid Registered Nurse (RN) license with current state certification.
- Proven experience in critical care settings such as ICU, emergency room, or trauma centers; experience in Level I or Level II trauma centers is highly desirable.
- Strong background in hospital medicine, medical-surgical units, or outpatient clinics; home health experience is preferred.
- Knowledge of health regulation policies, medical terminology, and infection control standards including HIPAA compliance.
- Demonstrated ability to perform comprehensive patient assessments and manage complex clinical cases effectively.
- Proficiency with electronic health records (EHR/EHR), nursing documentation, and utilization management systems.
- Excellent communication skills for client interaction and interdisciplinary collaboration; experience managing patients as a nurse case manager is a plus.
- Ability to handle diverse patient populations including geriatrics, pediatrics (PICU experience preferred), behavioral health patients, and those requiring hospice or post-acute care services.
- Flexibility to adapt to various healthcare environments such as inpatient hospital units or outpatient clinics; acute care experience is advantageous.
