What are the responsibilities and job description for the Emergency Services Case Manager, St. Louis position at Social Work p.r.n.?
A mission-driven organization is seeking a temporary (approximately 6 months beginning in November), full-time social worker to provide intake, assessment, and case management services to individuals seeing emergency assistance. Monday-Friday, one evening shift per week (Monday or Wednesday).
Key Responsibilities
- Perform intake and assessment with clients to determine eligibility, needs, and barriers, and facilitate goal-setting to work toward self-sufficiency
- Connect participants with resources to assist with basic needs such as food, utilities, rent/mortgage payments, transportation, medications, etc.
- Conduct follow-up case management meetings with clients to monitor and track progress toward goals
