495 - Case Manager

Social Vocational Services is a nonprofit organization, which for over 41 years has been committed to serving the needs of individuals with developmental disabilities. SVS takes pride in its dedicated staff and is currently seeking dependable, caring individuals to become part of its team.

Case Manager Position Purpose: To develop and monitor individual client plans within a day program office.

Duties and Responsibilities

· Oversee assessment of clients

· Assist in development of client objectives

· Ensure client health and safety issues are addressed at job site

· Set up and monitor client files

· Participate in client service planning meetings

· Write reports on client progress

· Meet with clients to discuss progress

· Communicate effectively with support teams

· Coordinate with consultant services, if any

· May be required to provide direct service to consumers

· May be required to assume Program Director duties in some circumstances

· Other duties as assigned by Program Director or Regional Director

· Mandated Abuse Reporter

** NEW HIRE BONUS - $750 hiring bonus after 60th day of full attendance (must work full shift to be counted towards the 60 days).

Driving and License Requirements: Must have personal vehicle, valid California Driver’s License, proof of auto liability insurance as mandated by State of California and a good driving record. May be required to drive 9 passenger van.

Criminal Record Clearance: Must obtain and maintain Department of Justice/FBI fingerprint clearance.
