What are the responsibilities and job description for the Case Manager for Seniors position at Social Service Staffing & Recruiting, Inc.?
This is temporary, full time, starts ASAP and can be temp to hire, the temp assignment is for at least 3 - 6 months. The hours are M - F basic business hours. The Case Manager functions as a geriatric care manager for frail low-income elders enrolled in the MSSP Medi-Cal Waiver program. Work is performed within established procedures. The CM evaluates the potential client as a total person and identifies the functional limitations that impede independent living. The CM plans, provides, and coordinates a wide range of services to assist clients in avoiding nursing home placement and to remain safely living independently. The CM works as a member of the team providing health and social services to frail elders and adults with disabilities to meet the agency's mission to create opportunities where people can learn, grow and age independently with dignity and community. There will be frequent travel to clients’ homes throughout West, Central and East Contra Costa County. The assignment is 60% in the office and 40% in the community, mileage is reimbursed.
Qualifications:
- MSW/ MA/ MS plus one year working with the elderly OR BA and two years experience working with the elderly.
- CDL, clean DMV record and automobile insurance.
- Must pass Department of Justice fingerprint clearance.
