The Job Transition Specialist supports the Certified Employment Specialist in managing and coordinating employment services for adults with developmental and intellectual disabilities. This role involves administrative and direct support duties to help with job placement, post-secondary education, job development and skills development. It is important for the JTS to be able to establish trust, guide clients and counsel them throughout the employment process.
Duties and responsibilities
- Assist the Certified Employment Specialist with Discovery, Exploration, situational observation, and assessments
- Maintain accurate records and documentation
- Develop personalized transition plans for clients
- Conduct assessments to determine the client's needs
- Provide emotional support and counseling
- Coordinate resources and services to aid in transitions
- Transport clients to and from job interviews
- Transport clients to and from a job, during the job shadowing period
- Assist with job applications if needed.
- Assist with job placement or educational transitions.
- Monitor and evaluate the progress of clients
- Advocate for clients' needs and rights
- Collaborate with Certified Employment Specialist, other professionals, and organizations.
- Develop and distribute informational materials pertaining to employment opportunities
- Support clients in setting and achieving goals
- Facilitate communication between clients and relevant parties, such as managers and coworkers at prospective jobs.
Qualifications
- High school diploma or equivalent
- Minimum one year of experience working with adults with developmental or intellectual disabilities.
- Job Coach Certificate or proof of TRN training with 12 hours of job shadowing.
- Strong communication and interpersonal skills
- Excellent organizational and time management skills
- Ability to work independently and as part of a collaborative team
- Completed Relias courses for Employment Services and DSP
- CPR and First Aid Certification
Working conditions
- This is a full-time position that may involve physical activities such as lifting, bending, and assisting clients with mobility.
- Flexible schedule including evenings, weekends, and holidays as needed.
- Will require traveling.
