Job Description: Direct Support Professional (DSP)
General Summary:
Responsible for the general health and welfare of supported persons living in the home; including but not limited to, food planning and preparation, toiletry and personal care, transportation from appointments and community outings, laundry, and other household chores.
Essential Functions:
- Follow Individual Support Plan (ISP), Person-Centered Support Plan (PCSP), Behavioral Support Plan (BSP) to ensure individualized needs are met and documented.
- Encourage and assist supported persons learn daily living skills as appropriate.
- Complete all documentation required by SOAR and governing agencies in a timely manner.
- Maintain a healthy, clean, safe living environment at all times.
- Administer medications as directed by the Medication Administration Record (Medical Certification Required).
- Safely transport supported persons to appointments and community outings.
- Assist and track supported persons personal spending by retaining receipts and accurately maintaining finance logs.
- Maintain confidentiality of records and information in accordance with state and federal guidelines (HIPPA).
- Report any incidents or behavioral concerns to the House Manager or Program Director immediately.
- Respect the rights of those supported.
Job Requirements:
- Maintain a valid Driver’s license
- If receiving mileage reimbursement for use of personal vehicle, proof of insurance is required.
- Compliance with Relias, CPR training and any other training required by SOAR and Department of Disability and Aging (DDA).
- Knowledge of documentation required for Reportable, Non-Reportable Event Forms and Medication Variance.
- Provide emergency coverage as requested by management.
Job Qualifications:
- Must be 18 years of age
- High School Diploma or equivalent
- Effectively read, write and communicate verbally
- Basic computer skills
· Experience in care-giving or working with intellectually/developmentally disabled adults preferred.
- Must pass state-required background check, drug/alcohol screening, Tuberculosis test, and not be listed on the Department of Health Abuse Registry or TN Sexual Assault Registry.
