Licensed Vocational Nurse (LVN) – Hospice (Field-Based)

Location: Los Angeles, CA (Field-Based)
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to provide direct patient care under the supervision of a Registered Nurse in a hospice setting. The LVN will follow established plans of care, deliver treatments, and collaborate with the interdisciplinary team to ensure quality end-of-life care.
Responsibilities:
- Provide direct patient care in accordance with the State Nurse Practice Act
- Manage and care for a caseload of 5 patients
- Implement and follow individualized plans of care
- Administer treatments and monitor patient condition
- Provide accurate and timely documentation of patient services
- Educate patients and families regarding care plans and disease process
- Assist in coordination of hospice services with the interdisciplinary team
- Support pain and symptom management and evaluate patient response
- Utilize medical equipment and supplies efficiently
- Participate in quality improvement and professional development activities
Qualifications:
- Graduate of an accredited LVN program
- Active California LVN license
- Hospice experience is required (MD/hospice setting experience preferred)
- Minimum of 1–3 years nursing experience (hospice, home health, or med-surg preferred)
- Strong communication and organizational skills
- Current CPR certification
- Valid driver’s license with insured and reliable transportation
- Understanding of hospice philosophy and care for terminally ill patients
- Bilingual is a plus (not required)
- PRN availability is a plus
Job Type: Full-time / PRN (Per Diem)
Pay: $50–$65 per visit
