Position Title:  MDS Coordinator    

FLSA Status: Exempt          

Reports To:    Administrator

 

General Description

The Resident Care Coordinator conducts and coordinates the completion of required Resident Assessment Instruments (RAI) and is responsible for the implementation and ongoing evaluation of each resident’s comprehensive plan of care.

Hours of Work

  • Monday-Friday, hours determined by the Administrator.
  • Required to work in office.
  • Included in on-call rotation as needed

  • Intermediate computer typing skills, and ability to navigate electronic medical record systems.
  • Ability to work independently, and to organize, plan, and manage time effectively to complete assignments.

Job Responsibilities, Duties and Authority

  • Coordination of the communities Resident Assessment Instrument (RAI) process in accordance with state and federal regulations.
  • Accurate completion of all MDS assessments and any supporting assessments or clinical documentation.
  • Implementation and ongoing evaluation of each resident’s comprehensive plan of care.
  • Scheduling interdisciplinary care plan team meetings.
  • Providing backup support for leading interdisciplinary care plan team meetings.
  • Auditing medical records for the purpose of supporting documentation for all items coded on the MDS. 
  • Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving about the SNF prospective payment system and quality reporting program.
  • Review staff documentation to ensure continuity of care and appropriate utilization of the Care Plan
  • Utilizes resources such as RAI manual and online CMS resources to ensure MDS’s, CAA’s, and care plans are following State and Federal guidelines.
  • Utilize computer system to produce all required documentation and reports.
  • Monitor resident care indicators including, but not limited to, pressure areas, chemical and physical restraints, and infections together with assigned nursing staff. 
  • Utilizes the current RAI Manual as a resource during the assessment coding process.
  • Schedules the Assessment Reference Dates (ARDs) for payment assessments in a manner that accurately captures each resident’s clinical characteristics for payment classification purposes.
  • Schedules the ARDs for all OBRA (Omnibus Budget Reconciliation Act) assessments daily, monthly, and as needed.   
  • Provides a schedule of ARDs and assessment types weekly and as needed to the interdisciplinary team (IDT) to facilitate the timely completion of MDS sections and CAA’s (Care Area Assessments) by each discipline.
  • Communicates with members of the IDT as needed for timely completion of assessments.  Expected to report any issues with timeliness to the Administrator.
  • Transmits assessments in accordance with current regulations.  To facilitate timely receipt of validation reports, will transmit as frequently (daily) as necessary to obtain timely validation of MDS acceptance into the Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP) System data base.
  • Confirms transmission files are accepted by review and printing of initial and final validation reports.
  • Corrects any fatal errors immediately and retransmits the assessment until an accepted validation report is received.
  • Ensures the Business Office designee receives the initial and final validation report that acknowledges the acceptance of the MDS into the QIES ASAP.
  • Facilitates the completion and updating of resident care plans with the IDT to reflect each resident’s current needs.
  • Communicates with the IDT to identify residents in need of a Significant Change in Status assessment, based on criteria in the current RAI manual.
  • Participates in all resource utilization and/or triple check meetings prior to billing of claims associated with payment assessments.
  • Participate with ILASN as required.
  • Treats all residents with dignity and respect.  Promotes and protects all residents’ rights.
  • Establishes a culture of compliance by adhering to all community policies and procedures.  Complies with standards of business conduct, and state/federal regulations and guidelines.
  • Always follows appropriate safety and hygiene measures to protect residents and themselves.
  • Maintains confidentiality of protected health information, including verbal, written, and electronic communications.
  • Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.  Reports any retaliation or discrimination to HR or compliance officer.
  • Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to the administrator.  Protects residents from abuse and cooperates with all investigations.

Meetings

  • Attends meetings as required.
  • Meets at least monthly with the Administrator.

Working Conditions

  • Must be able to spend time walking, standing, pushing, pulling, bending, reaching, lifting and sitting.
  • Must be able to lift more than 20 pounds 5-10% of the time.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to use a computer, phone, etc. for extended periods of time.
  • Exposure to blood or to body fluids, fumes, or airborne particles and toxic or caustic chemicals.
Qualifications:

Minimum Training, Experience and Certification Requirements

  • Must possess a current, unencumbered, active license to practice as a RN/LPN in this state. 
  • Must have 3 years’ experience as a RN/LPN, one of which includes supervisory experience.  Experience with MDS completion preferred.
  • Resident Assessment Coordinator (RAC) training must be completed within six months of hire.  RAC Certification is preferred.

Insurance Programs:

Health, dental, vision, short-term disability, and life insurance are available to all full-time employees. Insurances will begin on the 1st of the month following 30 days of employment.

Health:

There are 2 options for medical insurance, a PPO, and a High Deductible Health Plan (HDHP). The PPO offers a $1500 deductible with a max out-of-pocket of $4500. All preventative care services are covered at 100% by insurance. All generic prescriptions are a $10 copay. Most Carle providers would be in-network.

The HDHP plan offers $3,800 deductible with a max out-of-pocket of $6,100. All preventative care services are covered at 100% by insurance. Most Carle providers would be in-network. This plan also works with our Health Savings account.

Health Savings Account:

Snyder Village offers a health savings account through EPIC! If a staff member is contributing to the H.S.A. Snyder Village (SV) will also contribute to their balance. SV offers $0.50 for every $1 a staff member contributes up to $1300 per year.

Dental:

Dental coverage is through the Guardian network. Offering a deductible of $50 for in-network services and 100% covered preventative care services.

Vision:

VSP coverage is through Guardian but with the VSP network. Exams are a $10 copay with any in-network provider. Exams are covered once every 12 months, lenses once every 12 months, and frames are covered once every 24 months.

Basic Life and AD&D:

Any staff member who enrolls in medical coverage will receive $10000 in company-paid life insurance and AD&D through Guardian.

Voluntary Life Insurance:

Through Guardian staff members may select additional life insurance for themselves, their spouse, or their children. A staff member can select a policy for themselves in $25000, $50000, $75000, or $100000. Spousal coverage can be selected for up to 50% of employee coverage. Coverage for children can be selected for each child up to $10000.

Short-Term Disability:

Short Term Disability insurance is provided through AFLAC. The plan is designed to replace part of a staff members’ paycheck in the case of an illness, injury or surgery that prevents you from working according to the doctor’s orders. Staff members receive approximately 60% of their gross pay.

401k:

All staff members are automatically enrolled to the 401k retirement program through Empower on their 91st day of employment at 3% of their salary. However, Snyder Village will contribute and match an employee’s contribution by up to 5% on their salary. An employee is considered fully vested after 3 years of employment.

Paid holidays

Snyder Village recognizes seven holidays as listed below:

  • New Year’s Day
  • Easter Sunday
  • Memorial Day
  • Fourth of July
  • Labor Day
  • Thanksgiving
  • Christmas Day

Any non-exempt staff member working on the holiday will receive time and one-half of their hourly straight pay.

Paid Time Off

Paid Time Off (PTO) is offered to full-time and part-time workers who work more than 20 hours per week. All newly hired staff members accrue up to 2 weeks or 80 hours of PTO.

Paid Personal Days

All staff members regardless of classification accrue paid personal days (PPD). Each staff member accrues up to 40 hours of PPD.

Sick Pay

Once a full-time staff member has completed 1 year of employment, they are given 80 hours of sick time to use for a serious illness requiring more than 5 days off from work.

 

Tuition Reimbursement

Snyder Village values the professional and personal development of our staff members. Tuition reimbursement is available for education expenses such as course fees, technical training, certification programs, or other needed resources. Reimbursement will also be provided for successful certificate completion, up to six-course hours per semester.

Scholarship

Snyder Village provides up to 2 nursing scholarships per year to staff members pursuing an RN or LPN degree. Snyder Village also provides up to 4 C.N.A. scholarships per year to staff members who are pursuing a certified nursing assistant degree.

 

 

 

Salary : $78,700 - $92,500
