Responsibilities:
· Answer incoming referral office telephone, respond to electronic referral communication and faxes as needed for smooth and timely referral/admission processing
· Provide physicians, patients, family members and other callers with information about Snowline service lines
· When referral is received obtain initial referral information, coordinate patient evaluation by arranging date and time of meeting with patient and advise admission nurse(s) about appointment time. Provide admission nurse(s) other relevant referral information.
· Notifies referring physician office or agency about need for copies of medical records
· Enter initial referral/admission data into Electronic Medical Record (EMR)
· Coordinate admission packets with admission nurse
· Oversees agency’s patient-census
· Collaborates with agency marketing team to ensure facility waivers and required contracts are in place prior to hospice admissions
· Coordinates durable medical equipment (DME) for hospice patients as needed during the referral process
· Prepare Kaiser Diverted Patient Report, as required fax to Kaiser and maintains file copy for future reference
· Responsible for ensuring needed intake/admit documents are received and uploaded into EMR for teams to view and available
· Communicate and relay messages promptly and clearly
· Work collaboratively with the admission team and other relevant disciplines to ensure smooth program enrollment visits
· Perform required data entry into all admission platforms accurately and timely
· Provide exceptional customer service with staff, physicians, patients and all community referral partners.
· Demonstrate a proactive attitude, take the initiative, collaborate effectively with others, remain adaptable to change, and be reliable and accountable
· Demonstrate the capacity to engage constructively with diverse belief systems while respecting individual preferences, choices, and lifestyles
· Use discretion when relaying confidential patient information; understand and abide by HIPAA regulations
· Perform job responsibilities and duties in an efficient, accurate, and timely manner
· Work within scope of job description
· Electronic Medical Records (EMR) experience (preferred)
· Perform other related duties as assigned
Qualifications:
- High School Graduate or GED
- Four years general office and administrative support experience OR two years general office and administrative support plus an Associate Degree in business or related field
- One year experience in a healthcare setting preferred
- Referral Coordination Experience Required
- Knowledge of medical terminology
- Demonstrated ability to perform detailed work in a timely manner with accuracy and thoroughness
- Demonstrated ability to prioritize tasks, handle pressure and delicate situations while effectively carrying out job responsibilities
- Demonstrated computer skills with extensive experience using Microsoft Word and Excel required, Access preferred
- Must be able to rotate weekends with the rest of the team
