We are seeking a detail-oriented and tech-savvy Care Coordinator to perform CCM (Chronic Care Management) chart reviews. Key Responsibilities:
Review patient charts and EHR data to identify qualifying conditions for CCM billing
Document time spent and services provided in accordance with CMS guidelines
Generate clinical summaries and communicate findings to DONs as needed
Track and log CPT codes based on services rendered
Coordinate with internal teams and ensure timely and compliant billing
Use CCM software tools to manage patient populations and communication
Maintain HIPAA compliance and patient confidentiality at all times
Qualifications:
Experience in a healthcare or administrative support role (CCM or RPM experience a plus)
Strong attention to detail and critical thinking skills
Excellent written communication skills
Quick learner with ability to adapt to multiple software platforms
Familiarity with EHR systems, especially PointClickCare, Athena, or eClinicalWorks
