Smoky Ridge Health and Rehabilitation is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. The ideal candidate will have a passion for helping patients regain their strength, mobility, and independence following illness, injury, or surgery. This role involves evaluating patients, developing personalized treatment plans, and delivering high-quality, evidence-based therapy services to residents.
Key Responsibilities:
Perform comprehensive physical therapy evaluations and re-assessments
Develop individualized treatment plans based on patient goals and clinical findings
Provide direct physical therapy services in accordance with physician orders and facility protocols
Monitor and document patient progress, outcomes, and response to treatment
Collaborate closely with interdisciplinary team members including occupational therapists, speech-language pathologists, nurses, and physicians
Participate in care plan meetings and family conferences as needed
Educate patients, caregivers, and staff on exercise programs and mobility techniques
Maintain accurate and timely documentation in the electronic medical record system
Ensure compliance with all federal, state, and facility regulations and guidelines
Supervise and mentor Physical Therapy Assistants (PTAs) and support staff when appropriate
Qualifications:
Graduate of an accredited Physical Therapy program
Current state licensure as a Physical Therapist (or eligible)
CPR certification (BLS) required
Experience in a SNF or geriatric setting preferred but not required
Excellent communication, organizational, and interpersonal skills
Strong clinical reasoning and documentation skills
Ability to work effectively as part of a collaborative team.
Work Schedule:
As needed with occasional weekend or holiday coverage as requested.
