What are the responsibilities and job description for the Master Social Worker position at SMOKY MOUNTAIN HOME HEALTH & HOSPICE?
- Holds a Master's Level Degree from a Social Work curriculum accredited by the council on Social Work education, with at least one year experience in a medical setting.
- Knowledge of the framework, organization and function of a Home Health Care Program and knowledge of the principles and methods involved in planning and in directing ancillary personnel.
- Maximizes the social functions of each patient and the coping capacity of the patient's family.
- Fosters the human dignity and personal worth of each patient.
- Understands the social factors in the patient's day to day behavior including staff-patient relationships.
- Prepares the patient for changes in his living situation.
- Assists the family in developing constructive and personally meaningful ways to support the patient.
- Delivers Social Services in accordance with the approved plan of care and assists the patient and home caregiver in identifying and utilizing information and services from other community services.
- Evaluates or assists the attending Physician in evaluating the social and emotional needs of the patient and assesses the capacity of home caregivers to cope with the patients' needs in order to develop the plan of care.
- Completes all required paperwork associated with visits on a daily basis and submits in a timely manner.
- Writes supplement orders for SW frequencies after initial hospice evaluation.
- Prepares clinical and progress notes.
- Observes and reports to the attending Physician and other hospice staff involved in the patient's care, at least every 62 days, the patient's reactions to the treatments and any changes in the patient's physical, emotional or financial condition or plan of care.
- Maintains all records and information relevant to social services provided in accordance with the general policies of the hospice.
- Assists the Physician and other members of the health team in understanding the significant social and emotional factors related to the patient's health problems.
- Assesses the social and emotional factors in order to estimate the patient's capacity and potential to cope with problems of daily living by promoting the human dignity and personal worth.
- Helps the patient and his/her family to understand, accept and follow medical recommendations. Provides services planned to restore the patient to optimum social and health adjustments within his capacity. Assists the family to develop constructive and personally meaningful ways to support the patient.
- Assists patients and their families with personal and environmental difficulties which predispose toward illnesses, interfere with obtaining maximum benefits from medical care and prepares the patient for changes in his/her living situation.
- Utilizes resources, such as family and community agencies to assist patient to resume life in the community or to learn to live with his/her disability.
- Develop or participates in staff training programs and acts as a consultant to the staff.
- Participates in the hospice's quality assurance reviews and evaluations of social services, when applicable.
- Develops the patient's plan of care for social services and assisting the hospice staff, the patient and the home caretaker in understanding the plan of care.
- Conducts in-services as needed for agency personnel.
