- Hours: 8:00 AM – 4:00 PM
- Hours per week: 40 hrs
Requirements:
- 1 years of experience as a Social Worker in hospital setting
- Master of Social Work
- Social Worker License
- BLS
- Covid card
Responsibilities
- Conduct psychosocial assessments for patients and families
- Provide counseling, emotional support, and crisis intervention
- Participate in multidisciplinary rounds to share psychosocial insights
- Advocate for patients regarding care planning and advance care decisions
- Coordinate comprehensive discharge plans (SNF, rehab, LTAC, hospice, etc.)
- Connect patients with community resources and entitlement programs
Pay: $2,400.00 - $2,668.60 per week
