Schedule: 2 days on → 2 days off → 3 days on | Rotating weekends

Position Overview
We are seeking a compassionate LPN/LVN – Long-Term Care professional for a night shift, permanent position. The LPN works under an RN Supervisor and oversees daily resident care, medication administration, and NA supervision.
Ideal for candidates passionate about elderly care in a supportive, community-focused environment.
Key Responsibilities
- Administer medications, treatments, and glucose checks
- Accurately transcribe and follow physician orders
- Complete monthly summaries and update care plans
- Notify RN Supervisor of any changes in resident condition
- Assist with rounds, examinations, and transfers
- Collect lab specimens and document in PCC (EHR/EMR)
- Participate in post-mortem procedures and family communication
- Supervise Nursing Assistants and assist with evaluations
- Complete incident reports, weight checks, and vitals
Required Qualifications
- Active PA LPN license or proof of application at interview
- BLS (AHA)
- Knowledge of infection control, medical asepsis, body mechanics
- Strong communication documentation skills
- Ability to follow written & verbal instructions
- Professional, courteous behavior with residents & families
