Qualifications as a Physical Therapist:

• Must have a Degree in physical therapy and a valid Physical Therapist NYS license
• We are looking for a Physical Therapist PT who has the desire to implement high-quality Physical Therapy services. Looking for a Physical Therapist who is strong in identifying patient needs in long-term residents, a team player, and efficient with their time.

Physical Therapist Responsibilities:

• Must have 1 year SNF experience
• Performing evaluations/screens, establish patients goals, develops individualized treatment plans, developing treatment and discharge plans
• Provide quality physical therapy services encompassing personalized and evidence-based care and proven interventions
• Documenting progress toward identified objectives
• Overseeing Physical Therapy Assistant(s) and provides onsite supervision of treatment interventions and clinical documentation
• Developing and implementing new program/techniques to enhance physical therapy services.
• Conducting patient and family education
• Independently manage workflow which includes allotting time for treatment documentation of individual treatments and notes in the electronic records system
