Full Time Role
Salary: 65,000 to 70,000 (Commensurate with experience)
5 days per week schedule
On-call manager rotation
POSITION PURPOSE:
To support the activities of the Social Services department while also assisting with the Admissions processes on an as needed basis.
Social Services Manager
- Attends PAN meetings to engage actively in discharge planning.
- Serve as a liaison with other departments for discharge planning.
- Track status and progress of appeals.
- Attend and co-lead care plan meetings.
- Manage and coordinate goods and services with off-site companies for durable medical equipment, home health visits, and other services required for discharge.
- Assist in the oversight of the LTC unit.
- Mentor, train, and provide supervision to other assistants.
- Serve as acting Director in the absence of the Director.
Requirements:
- Bachelor’s degree in social work, Healthcare Administration, or related field
- LSW preferred
- 3 years’ experience providing resident or social service support.
- 1 year supervisory experience
