Job Description The Diabetes Educator, in collaboration with the multidisciplinary teams, provides diabetes self-management training and education to patients with diabetes and their families. This includes coordinating care, assessing risk factors, providing individual and group education sessions, developing care plans, monitoring and communicating outcomes. CDCES required within 2 years of hire. Assessment of patients abilities, limitations and barriers to diabetes self-management Collaboration with referring provider and patient care team to identify and prioritize strategies to assist patients with diabetes self-management education Assist patients with medication adjustments to achieve optimal glycemic control in direct collaboration with the primary care provider Understand the pathophysiology of diabetes, medication management, acute and chronic diabetes related complications and motivational interviewing Use self-management training techniques to assess, plan and implement patient and family education in individual and/or group settings. Education to include self-monitoring of blood glucose, medications, diet/meal planning, exercise, goal setting, coping skills, and reduction of risk factors Keep current in new and advancing diabetes related technologies Participate in CQI projects and evaluation of outcome measures to assess diabetes education effectiveness Job Requirements Applicable Experience: 1 year Basic Life Support - American Heart Association or Red Cross, Cert Diabetes Care and Education Specialist - National Certification Board of Diabetes Educators, Registered Nurse (KS) - National Council of State Boards of Nursing (NURSYS), Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS) Bachelor's Degree - Nursing Job Details Full Time Day (United States of America) The best place to get care.
