Position Summary:

Sleep & Airway Medicine Centers is seeking a dedicated and patient-focused Nurse Practitioner to join our multidisciplinary sleep medicine team. This role is responsible for evaluating, diagnosing and managing patients with sleep-related conditions while providing comprehensive education regarding available treatment options.

In addition to traditional sleep medicine responsibilities, the Nurse Practitioner will play an important role in supporting SAMC’s Durable Medical Equipment (DME), PAP therapy, and Oral Appliance Therapy (OAT) pathways. The NP will help patients understand their diagnosis, review appropriate treatment options, provide education regarding recommended therapies, and support patients as they transition into the appropriate treatment pathway.

The ideal candidate is comfortable discussing multiple treatment modalities in an objective, patient-centered manner and collaborating across medical, dental, DME, and operational teams to ensure patients understand their options and receive appropriate follow-up care.

Key Clinical Responsibilities:

• Perform comprehensive clinical assessments, including intraoral examinations, patient histories, review of symptoms, and sleep-specific evaluations.
• Review and interpret appropriate diagnostic testing, including home sleep studies, in-lab polysomnography, oximetry, and related testing as appropriate.
• Evaluate patients diagnosed with sleep-disordered breathing and other sleep-related conditions.
• Develop individualized treatment plans based on clinical findings, diagnostic results, patient needs with physician recommendations.
• Initiate and monitor appropriate therapies, including PAP therapy, Oral Appliance Therapy (OAT), behavioral interventions, and other treatment modalities within the provider’s scope of practice.
• Provide ongoing follow-up care and evaluate treatment effectiveness, tolerance, adherence, and clinical outcomes.
• Document all patient encounters, education, recommendations, and treatment decisions accurately and timely within the EMR.

DME / PAP Therapy Responsibilities:

• Educate patients regarding PAP therapy, including CPAP, APAP, BiPAP, and other prescribed PAP modalities when appropriate.
• Explain the purpose of PAP therapy, expected benefits, general treatment expectations, and the importance of adherence.
• Review PAP therapy as a treatment option with patients and answer clinical questions regarding the recommended therapy.
• Provide education regarding common barriers to PAP adherence and available resources for addressing treatment concerns.
• Reinforce the importance of ongoing follow-up, compliance monitoring, and communication with the patient's DME provider and clinical care team.
• Coordinate with the appropriate DME team to ensure patients are transitioned into the DME process following the clinical recommendation or prescription.
• Support clinical documentation and orders necessary for DME coordination within the NP's scope and established SAMC workflows.

Oral Appliance Therapy (OAT) Responsibilities:

• Educate appropriate patients regarding Oral Appliance Therapy as a potential treatment option for obstructive sleep apnea and sleep-disordered breathing.
• Explain how OAT works, general treatment expectations, the importance of adherence, and the need for appropriate dental evaluation and follow-up.
• Discuss OAT alongside other clinically appropriate treatment options so patients can make informed decisions regarding their care.
• Identify patients who may be appropriate for OAT evaluation and facilitate referral or handoff to the appropriate SAMC dental provider or treatment team.
• Reinforce the importance of appliance follow-up, titration, treatment monitoring, and post-treatment sleep testing when ordered by the treating provider.
• Collaborate with dentists, treatment coordinators, and clinical team members to support continuity between the patient's medical diagnosis and dental treatment pathway.

Patient Education & Treatment Options:

• Provide clear, unbiased, and patient-friendly education regarding available treatment options.
• Help patients understand their sleep study results, diagnosis, health implications, and recommended next steps.
• Review the differences between PAP therapy, Oral Appliance Therapy, and other available treatment pathways when clinically appropriate.
• Answer patient and caregiver questions regarding treatment expectations and the importance of treating sleep-disordered breathing.
• Utilize shared decision-making principles while ensuring recommendations remain consistent with the patient's clinical needs and the treating provider's plan.
• Reinforce treatment recommendations without creating pressure toward a particular therapy solely for operational or financial reasons.
• Document treatment education and the patient's selected or recommended next steps.

Care Coordination & Collaboration:

• Collaborate closely with sleep physicians, dentists, dental assistants, treatment coordinators, DME partners, and front-office teams.
• Ensure clear clinical handoffs when patients transition between diagnostic, DME/PAP, OAT, and follow-up phases of care.
• Participate in multidisciplinary case reviews and care-team meetings as needed.
• Communicate barriers to treatment, patient concerns, or clinical issues to the appropriate team member.
• Support consistent patient education and clinical workflows across SAMC locations.
• Assist with identifying opportunities to improve patient adherence, treatment acceptance, and continuity of care.
• Maintain appropriate separation between clinical recommendations, patient education, DME fulfillment, and financial discussions according to SAMC workflows.

Qualifications

• Active state licensure as a Nurse Practitioner.
• National Board Certification through AANP or ANCC.
• Current BLS certification; ACLS preferred but not required.
• Minimum of one year of clinical experience required.
• Experience in sleep medicine, internal medicine, family practice, pulmonology, ENT, or a related specialty preferred.
• Experience with sleep-disordered breathing, PAP therapy, DME, or Oral Appliance Therapy is highly desirable.
• Strong patient-education and communication skills.
• Ability to explain complex clinical information in a clear and approachable manner.
• Strong interdisciplinary collaboration skills.
• High level of comfort with EMR systems and digital clinical workflows.
• Independent clinical judgment, critical thinking, and a proactive approach to patient care.

Work Schedule & Clinical Expectations:

Schedule: Monday through Thursday, 8:00 AM – 5:00 PM

Patient Volume: Up to 24 patients per day, including new consultations, follow-ups, treatment education, and therapy management visits

Environment Rounding: In-person outpatient clinical setting across SAMC locations

On-Call: None
