- Serve as the primary point of contact for patient inquiries and admissions
- Coordinate with healthcare providers, referral sources, and insurance companies to facilitate the admissions process
- Should have Insurance experience
- Conduct patient assessments to determine eligibility and develop personalized care plans
- Collaborate with the care team to ensure a comprehensive approach to patient care
- Maintain accurate and up-to-date records of patient admissions, assessments, and treatment plans
- Process admission paperwork
- Develop and implement effective admissions strategies to meet or exceed departmental targets

- Minimum of 2 years of exeperience in a Skilled Nursing Facility

- Familiarity with healthcare regulations
