Your Schedule:
- Monday – Friday (Flexible Days)
- 9:00 AM - 3:00 PM (total about 10 - 12 hours / week)

Your Responsibilities:
- Must care deeply and passionately for the well-being of our senior population.
- Member / Client Assessments For Both Adult Day Care Center & In-Home care
- Reviewing health care documentation,
- Being a resource for the Skylark team,
- Assisting in the operations of the center when needed
- Review and complete monthly, weekly required nursing documentation for CCSP / SOURCE, Veterans Administration programs, and compliance requirements
- Care plan development.
