Job Summary
Behavioral Health Case Manager is responsible for providing case management services to clients in residential and/or community-based programs. This position coordinates care, advocates for client needs, connects clients and families to resources, and monitors service delivery to ensure treatment goals are met. The Case Manager works collaboratively with the treatment team, community partners, and funding agencies to promote client stability, recovery, and overall well-being.
Responsibilities
- Serve as primary contact for assigned caseload, ensuring each client’s individual service plan is implemented and monitored.
- Conduct intake assessments, gather relevant background information, and identify client strengths, needs, and barriers.
- Coordinate and schedule services including therapy, psychiatric care, medical appointments, educational support, and community resources.
- Maintain ongoing contact with clients, families, legal custodians, and service providers to track progress and address needs.
- Facilitate and attend treatment team meetings, service plan reviews, and court hearings as required.
- Provide advocacy and assist clients/families in navigating systems such as healthcare, education, and social services.
- Maintain detailed, accurate, and timely documentation in accordance with agency policies and applicable regulations.
- Assist clients with developing life skills, self-advocacy, and community integration strategies.
- Monitor aftercare and discharge plans to ensure continuity of care.
- Participate in on-call rotation if required by the agency.
Required qualifications
- QMHS/CDCA
- High School Diploma
- Valid Drivers license
Preferred Qualifications
- Three years of relevant experience in mental health or substance use.
- Bachelor degree in related field.
- CPR and First Aid training.
- Crisis Prevention/De-escalation training.
Job Types: Full-time, Part-time, Contract
Pay: $20.00 - $24.00 per hour
