Position Overview
The Hospital Liaison plays a key role in ensuring census goals are met by overseeing the admissions process and supporting the center’s marketing action plan. This individual will focus on building strong referral relationships and facilitating smooth, timely admissions.
Key Responsibilities
Report directly to the Administrator
Achieve targeted admissions goals through effective execution of the marketing action plan
Ensure census mix goals are met or exceeded
Obtain referrals and gather complete clinical and financial information for admission review
Verify financial information prior to admission to support accurate billing
Build and maintain strong relationships with referral sources, including hospitals, physicians, case managers, and community partners
Conduct tours for prospective residents and families with a focus on closing
Support documentation, follow-up, and overall organization of referral and admissions activity
Qualifications
RN or LPN license preferred
2–3 years of clinical experience in admissions within a SNF, subacute, rehab, or healthcare facility
Proven ability to generate and convert referrals within healthcare markets
Strong organizational skills with attention to detail
Professional communication and telephone skills
Inside or outside sales experience preferred
Experience with Medicare and Medicaid guidelines
Proficiency with marketing information systems
