About the Role:

Join Skilled Nursing Pharmacy as a Staff Pharmacist and play a vital role in delivering exceptional pharmaceutical care to our patients. In this exciting position, you will collaborate with healthcare professionals to ensure optimal medication management and patient outcomes.

Responsibilities:

• Review and verify medication orders for accuracy and appropriateness.
• Provide clinical consultations to healthcare providers and patients.
• Monitor patient medication therapy for effectiveness and safety.
• Prepare and dispense medications in accordance with legal and regulatory standards.
• Conduct medication therapy management and patient education sessions.
• Maintain accurate records of prescriptions and patient interactions.
• Collaborate with interdisciplinary teams to optimize patient care.

Requirements:

• PharmD degree from an accredited pharmacy school.
• Active pharmacist license in California.
• Minimum of 1-2 years of pharmacy practice experience preferred.
• Strong clinical knowledge and ability to interpret medication therapy.
• Excellent communication and interpersonal skills.
• Detail-oriented with strong organizational abilities.
• Proficient in pharmacy management software and technology.
