About the Role:
Join Skilled Nursing Pharmacy as a Pharmacist, where you will play a vital role in providing exceptional pharmaceutical care to our patients. With a focus on improving health outcomes, you'll collaborate with a dedicated team in a supportive environment that values your expertise.

Available Schedule:

• PM Shifts starting times varies 4:00pm - 8:00pm.
• PM Shifts will include a Shift Differential.

Responsibilities:

• Review and interpret medication orders for accuracy and appropriateness.
• Provide drug information and counseling to patients and healthcare professionals.
• Monitor patient medication therapy for safety and efficacy.
• Participate in interdisciplinary team meetings to optimize patient care.
• Ensure compliance with state and federal pharmacy regulations.
• Prepare and dispense medications accurately and efficiently.
• Conduct medication reconciliation to prevent errors.
• Educate staff on medication management best practices.

Requirements:

• PharmD degree from an accredited pharmacy school.
• Active California pharmacist license.
• Minimum of 2 years of experience in a clinical pharmacy setting.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a fast-paced environment.
• Knowledge of pharmacy software and technology.
• Commitment to patient-centered care and safety.
• Detail-oriented with excellent organizational skills.
