About the Role:
Join Skilled Nursing Pharmacy as a Pharmacist, where you will play a vital role in providing exceptional pharmaceutical care to our patients. With a focus on improving health outcomes, you'll collaborate with a dedicated team in a supportive environment that values your expertise.
Available Schedule:
- PM Shifts starting times varies 4:00pm - 8:00pm.
- PM Shifts will include a Shift Differential.
Responsibilities:
- Review and interpret medication orders for accuracy and appropriateness.
- Provide drug information and counseling to patients and healthcare professionals.
- Monitor patient medication therapy for safety and efficacy.
- Participate in interdisciplinary team meetings to optimize patient care.
- Ensure compliance with state and federal pharmacy regulations.
- Prepare and dispense medications accurately and efficiently.
- Conduct medication reconciliation to prevent errors.
- Educate staff on medication management best practices.
- PharmD degree from an accredited pharmacy school.
- Active California pharmacist license.
- Minimum of 2 years of experience in a clinical pharmacy setting.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced environment.
- Knowledge of pharmacy software and technology.
- Commitment to patient-centered care and safety.
- Detail-oriented with excellent organizational skills.
