A skilled nursing and rehabilitation facility in the Clifton Springs area is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our care team. Duties:
- Collect and record resident health information, including vital signs and medical details
- Administer medications and injections as prescribed by providers
- Provide wound care and assist with dressing changes and other treatments
- Supervise Certified Nursing Assistants (CNAs) as needed
- Support residents with personal care and daily living activities
- Monitor and document residents’ nutritional and fluid intake/output
- Collaborate with the nursing team to maintain the highest standards of care
Requirements:
- Valid New York State LPN license in good standing
- Strong teamwork and communication skills
- Compassionate approach to resident care
- Prior long-term care experience preferred
