Now Hiring: Social Worker (SW) – Bronx, NY
DUTIES:
Maintain and/or improve residents’ ability to manage everyday physical, mental, and psychosocial needs
Identify individual social and emotional needs
Develop and maintain individualized care plans
Participate in planning for admissions, community re-entry, or transfers to other facilities
Collaborate in interdisciplinary care plan meetings to address residents’ social and emotional needs
REQUIREMENTS:
Bachelor of Social Work (BSW) or Master of Social Work (MSW) required
Degree must be in Social Work (candidates with Human Services or Psychology degrees will not be considered)
Long-term care or subacute experience required
