Qualifications
Experience working in a Long Term Care setting required
Strong computer and technical skills
Ability to communicate tactfully and effectively with residents, families, staff, and community partners
Self-starter with the ability to make independent decisions
Patient, enthusiastic, and professional demeanor
Essential Duties & Responsibilities
A. Referral Process
Conduct tours and interviews for prospective residents and families
Communicate with hospitals, liaisons, and community partners regarding referrals
Review referrals to ensure resident needs can be supported
Coordinate admission approval process
Collaborate with Administrator and Nursing in room assignment
Communicate admissions, discharges, and transfers to all relevant departments
B. Admission Process
Complete registry and criminal background verifications
Greet and assist all new admissions upon arrival
Educate residents and families on facility expectations, policies, and services
Maintain accurate resident roster
C. Resident Financial Services
Secure signed admission documents and financial paperwork
Assist with collections and participate in accounts receivable meetings
Support Medicaid application process when necessary
Complete reports (e.g. Representative Payee)
Ensure all documentation is uploaded and labeled correctly in PCC
D. Marketing & Relationship Management
Review quality data to support marketing strategies
Maintain strong connections with hospitals, liaisons, and community agencies
Promote exceptional customer service throughout the facility
Additional Responsibilities
Participate in QAPI initiatives
Comply with all facility policies and procedures
Collaborate with all departments to support admissions and census goals
Perform other duties as assigned
