Key Responsibilities
Social Services:
Support residents and families with emotional, social, and discharge planning needs
Advocate for residents’ rights and well-being
Coordinate care plans and participate in interdisciplinary team meetings
Assist with admissions, discharges, and transitions
Activities:
Plan, organize, and implement engaging recreational and therapeutic programs
Develop activities that meet the physical, emotional, and cognitive needs of residents
Encourage resident participation and community involvement
Maintain activity calendars and required documentation
Qualifications
Experience in long-term care preferred
Background in social services, human services, or related field
Previous activities experience or certification (preferred)
