What are the responsibilities and job description for the MDS Coordinator (LPN) position at Skilled Nursing and Rehab Care?
Responsibilities and Purpose • Collect all required data for MDS, including admission, quarterly, due to change of condition, etc.
- Mainatin timely and accurate MDS submissions
- Work with DON and team to update Care Plans as appropriate
- Electronically record and transmit MDS data according to the state database as required by state and federal regulations.
- Great communication skills, organized, and teamwork approach.
- Complete other duties as required and as given by Admin/DON
Desired Experience and Education • Current LPN License.
- Knowledge of the MDS and all associated tasks
- Preveious MDS Coordinator experience preferred
