Youth Transition Coordinator
Located in Parsons SKIL Office
Summary
Provide and coordinate Independent Living Services to youth transitioning from high school, a facility or an institution to live independently within the community of their choice and empower those individuals to reach self-directed goals. Assist youth in individual and group settings, both at the Center and in the individuals’ home and community with person-centered plans, connect youth to community resources, and build self-advocacy skills. Work closely with schools, families, and agencies who are involved in helping the youth with decision-making, goals and developing independent living skills.

Essential Job Functions
- Provide the five core services: Individual and Systems Advocacy, Independent Living Skills Training, Peer Counseling, Information and Referral and Deinstitutionalization.
- Assist youth with community resources such as: Housing, Transportation, Jobs and other resources as needed.
- Develop and update Independent Living Plans with youth, focusing on strengths, interests and individual goal setting.
- Provide assistance in identifying and coordinating existing services that will assist in achieving the independent living (IL) goals established in the Independent Living Plan.
- Teach independent living (IL) skills and job readiness skills, such as effective communication, teamwork and problem-solving.
- Identify barriers to integration within the community and actively pursue the removal of those barriers through the Agency’s ADA Advocacy Implementation Plan.
- Work with youth’s support network team as needed when assisting youth in developing a person-centered plan and achieving desired goals.
- Empower youth to identify their own needs, gain problem-solving skills and advocate for themselves.
- Work as a liaison between schools, families, employers and agencies involved in helping the youth.
Job Requirements
- At least 18 years of age
- Have a valid Driver's License
- Be familiar with Independent Living philosophy
