The Clinical Dietician works with patients and the clinical team to provide personalized nutrition recommendations to promote patient healing; educating the patients and families with practical dietary suggestions, being mindful of patient capacity for growth in the context of their cultural and emotional environment. Required Qualifications: BS Dietetics; Registered Dietitian credentials; Licensed State of New Mexico Ability to work individually and in a team Self-directed Ability to prioritize Excellent verbal and written communication skills Ability to instruct and coach staff Excellent customer service skills Duties and Responsibilities: Nutrition Consults: Nutrition Consults are addressed within the day following consult request and consult completion is documented in the electronic medical record (EMR) Nutrition Assessments: Nutrition Assessments for San Juan Regional Medical Center (SJRMC) and San Juan Regional Rehabilitation Hospital (SJRRH) patients identified at nutrition risk based on the Nutrition Screen or the Malnutrition Screening Tool are completed within two days and are entered into the EMR Follow-up Evaluations: The plan of care for patients identified as at nutrition risk is re-evaluated and documented within at least 1 week of the initial assessment and as necessary based on the plan of care Nutrition Education: Provide nutrition education to inpatients and outpatients at SJRMC, SJRRH, and the Cancer Center to facilitate self-management of medical nutrition therapy Nutrition Classes: Design and present classes (e.g. cardiac, diabetes, pulmonary, weight management) with current nutrition information for patients, providers, and graduate nursing students. Provide education to patients/caregivers at a level that that is understandable to them. Document education in the EMR within 24 hours of service. Outpatient Medical Nutrition Therapy: Provide education to outpatients at a level that that is understandable to them. Provide documentation of individual medical nutrition therapy to provider office within one week. Community Nutrition: Develop and present timely nutrition topics based on community need. Coordinate with marketing if appropriate and complete the community report form. Committees: Attend committee meetings as an active participant, report progress to clinical team, and serve as an education resource to other staff Clinical/IT: Participate in projects involving clinical and information technology overlap; assist with staff training as assigned Telephone Inquiries: Respond in a timely manner to phone inquiries. If by phone message attempt to return the call within 24 hours of receipt CDR Registration & State Licensure: Enhance professional growth and development to maintain current working knowledge, CDR registration and state licensure Department Education: Attend department in-services and meetings. Participate in QA/PI programs when assigned HealthStream: All annual HealthStream modules 100% completed Each employee is responsible for implementing SJRMC’s Service Standards into their daily work: Safety, Courtesy, Effectiveness, and Stewardship Other duties as assigned (demonstrates willingness and follows through with other duties requested) Physical Demands and Environmental Work Conditions: Mostly completing paperwork Extensive walking, sitting, and standing required As you go about fulfilling this mission, your work habits and work relationships should embody SJRMC's values.
