Care Coordinator - Sickle Cell Disease Program
Southern Jersey Family Medical Centers, Inc. is looking for a talented Care Coordinator to provide care and help make an impact in our fast-paced facility.
Summary: The Care Coordinator provides assistance to the physicians and nursing staff by coordinating care and services for patients with sickle cell disease and sickle cell trait in order to maximize the use of available resources and to offer complete preventative care to those in need.
Hours: This is a full-time position that requires the ability to work a flexible schedule. The position is based out of Burlington City but will provide coverage to regionally across Burlington County. Some travel between Salem & Atlantic County may also be required. General daytime hours Monday - Friday with 1 to 2 evenings per week. Weekends, based on event schedule. This is a grant position.
Position Responsibilities/Duties:
• Assist in coordinating patient care for visits, ordered tests, and
other healthcare needs
• Collaborate with providers and nursing
staff to identify concerns that would prevent quality medical
care
• Assist with patients enrolled in case management
•
Provide effective and productive communication between patients and
providers
• Educate and assist patients in the management,
prevention, and maintenance of their health wellness state through the
use of evidence-based guidelines and best practices
• Assess and
identify actual and potential patient problems by gathering relative
data
• Address gaps in the care of patients as identified in the
electronic health record
• Work with physicians and nursing staff
to develop a plan of care for patients and make adjustments as
needed
• Build and maintain relationships with both internal and
external healthcare contacts and organizations in an effort to improve
available patient resources
• Track patient outcomes and provide
information to authorized personnel as requested
• Maintain
electronic medical records (EMR) and other company information and
ensure compliance with all State and Federal guidelines
