JOB RESPONSIBILITIES:

  1. PREVENTION PROGRAM ACTIVITIES
    1. Coordinate and conduct outreach activities aligned with HIV Prevention and Harm Reduction best practices and community needs.
    2. Utilize the findings of multiple data and needs assessments to identify a comprehensive outreach strategy for conducting outreach in priorities areas, organizations and among diverse populations.
    3. In collaboration with the communications team and others, ensure the promotion and regular updates of Ryan White and HIV related information, current upcoming events, and necessary updates are posted online and/or promoted.
    4. Identify and conduct testing for individuals at high risk for HIV infection, increase education and awareness about prevention services and facilitate access to Counseling, Testing and Referrals (CTR).
    5. Provide CTR services for all clients during office visits or during outreach activities. Outreach activities may take place during evenings or weekends.
    6. Conduct Social Determinants of Health (SDOH) assessments at the prevention center or during outreach activities and assist navigation of resources within Sixteenth Street and other community agencies. Navigation services may include the provision of education, translation/interpretation, and other support based on client needs to facilitate access to services.
    7. Maintain a strong understanding of services and resources available to access medical care, treatment and other services including linkage to care, case management, and other resources in-house or at other agencies.
    8. Provide service to any individual visiting the Prevention Center, including Naloxone training and distribution, Syringe Exchange Program, safe use kits, and hygiene kits, among other services and materials available.
    9. Maintain files for each client participating in prevention interventions complying with State protocols and complete required data entry in the appropriate platforms.
    10. Complete all grant required and prevention training, and stay up to date on trends regarding HIV, HCV and STIs prevention, and other trends as identified by the Prevention Program Manager.
    11. Participate in the annual strategic planning process, the preparation of grant applications and reports, data collection efforts and other administrative assistance as determined appropriate by the VP of Community Health.
    12. Work closely with other members of the prevention team to successfully achieve the goals of the department while maintaining the organization of the Prevention Center to offer an inviting environment to our clients, this may include keeping supply inventories, procuring the cleanliness of office and work areas, and contributing with ideas for improvement of customer service experience.
2.  COMMUNITY ENGAGEMENT
  1. In collaboration with prevention program staff and other SSCHC staff, as needed:
    1. Connect with community partners, businesses, other stakeholders, and residents to develop partnerships and identify/create opportunities for collaborations.
    2. Maintain consistent familiarity with community resources and engage in meetings to better understand options to facilitate access for our clients.
    3. Monitor outcomes of referrals and assist in evaluating patterns of barriers that clients may face when attempting to access resources and work toward resolution as appropriate.
    4. Facilitate and/or participate in community events and community advisory board meetings.
QUALIFICATIONS:
  1. Bachelor’s degree with a minimum of one year experience with programs that reach and engage diverse populations, including people living with and affected by HIV, substance use disorder, community-driven interventions, or a combination of these. Consideration will be made of a suitable combination of previous academic and professional experience.
  2. Excellent oral and written communication skills. Bilingual (Spanish/English) fluency strongly preferred.
  3. Experience with collaboration, communication, engagement, and networking with organizations, leaders, and communities/peers representing various ages and diverse ethnic and cultural backgrounds.
  4. Excellent organizational skills and ability to work independently and in a group setting.
  5. Proficiency with MS Office, experience with MS Access or similar database platforms.
  6. Valid Wisconsin driver’s license, use of an automobile and current automobile insurance.???????
