Description

The Quality Specialist – PCMH/PCSP Consultant supports practice transformation and continuous quality improvement initiatives across Department of Pediatrics clinical sites. This role provides subject matter expertise in National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) and Patient-Centered Specialty Practice (PCSP) standards, guiding teams through recognition, compliance, and sustainment activities.

The position applies systems-based improvement methodologies to enhance clinical quality, patient experience, population health outcomes, and operational efficiency. The specialist collaborates with interdisciplinary teams to design, implement, and evaluate data-driven interventions that align with organizational and regulatory priorities.

Examples of Duties

Administration 80%

PCMH/PCSP Program Leadership

Serve as internal consultant on NCQA PCMH and PCSP standards, requirements, and updates

  • Lead and support practices through initial recognition and annual reporting processes
  • Conduct gap analyses and readiness assessments; develop action plans for compliance
  • Maintain documentation and evidence required for NCQA submission

Quality Improvement & Systems Support

  • Apply structured improvement methodologies (e.g., PDSA cycles, Lean principles) to drive measurable outcomes
  • Facilitate interdisciplinary teams in workflow redesign, care coordination, and performance optimization
  • Promote systems-thinking approaches to quality, safety, and patient-centered care

Data Analytics & Reporting

  • Collect, analyze, and interpret clinical quality metrics (e.g., HEDIS, UDS, internal dashboards)
  • Develop and maintain performance reports to track progress toward organizational goals
  • Translate data into actionable insights for providers, leadership, and care teams

Population Health & Care Coordination

  • Support risk stratification, care management workflows, and registry development
  • Collaborate with care teams to improve outcomes for high-risk and chronic disease populations
  • Align initiatives with value-based care and population health strategies

Collaboration & Communication

  • Partner with clinical leadership, IT, compliance, and operations teams
  • Facilitate meetings, communicate progress, and present findings to stakeholders
  • Foster a culture of continuous quality improvement and accountability

Education and Training 5%

  • Provide education and training to providers, residents, nursing staff, and care teams on NCQA PCMH/PCSP standards and quality improvement methodologies
  • Develop training materials, tools, and job aids to support practice transformation and sustainment
  • Serve as a subject matter expert and mentor for staff engaged in quality improvement initiatives

Academic & Professional Development

  • Contribute to the academic mission of SIU School of Medicine through participation in educational activities, curriculum support, or presentations
  • Support dissemination of quality improvement work through presentations, reports, or scholarly activity when applicable

Other Duties 5%

Performs other duties as required or assigned which are reasonably within the scope of the duties described above.

Qualifications

Credentials to be Verified by Placement Officer

  • Bachelor's degree in nursing, healthcare administration, public health, or related field (BSN preferred)
  • One (1) year (12 months) of Quality Improvement work experience in a clinical or healthcare related field.

Preferred Qualifications

  • Current, active Registered Nurse (RN) license in the State of Illinois (or eligibility for licensure)
  • Three (3) years of clinical nursing experience, preferably in ambulatory, primary care, or specialty practice settings

Knowledge, Skills & Abilities (KSA’s)

  • Knowledge of computer skills including Word, Excel, and PowerPoint
  • Knowledge of process improvement methodology and tools
  • Knowledge of data collection methodology
  • Knowledge of basic statistical process control components
  • Knowledge of benchmarking tools and best practices
  • Problem-solving and team facilitation skills
  • Skill using CQI tools and techniques
  • Strong analytical skills
  • Ability to train, present to small and large groups
  • Ability to act as a consultant to clinicians and non-clinicians
