This position is responsible for providing direct patient care to patients of the Center and for assisting
in teaching activities as they relate to all learner programs under the Center.
This position will report to the Director of Clinical Operations of the SIU Center for Family Medicine-
Decatur who in turn reports to the CEO/Executive Director for the FQHC.
Examples of Duties
I. TEACHING (10%)
Provide prevention and health maintenance care to patients with stable clinic
illnesses and perform interim physical examinations for healthy individuals
and families at the Center for Family Medicine and its offsite clinics.

- Teach and evaluate medical students, residents, physician assistants, and nurse
- Role model provision of care by a nurse practitioner focusing on health education,
- Attend Resident Core Conferences and assist in presenting as requested.
Provide prevention and health maintenance care to patients with stable clinic
illnesses and perform interim physical examinations for healthy individuals
and families at the Center for Family Medicine and its offsite clinics.
- Development of a collaborative clinical practice model with faculty and community
- Maintain competence in clinical practice using continuing education opportunities and
- Provide continuity of care across the health care continuum for collaborative practice
- Take part in Center Peer Review and Quality Improvement activities.
- Provide emergency services to patients as needed including those who are identified as
- Attend faculty meetings and resident conferences.
- Serve on the Center-wide Committees, as assigned.
- Serve on school and department committees, as assigned.
- Participate in community outreach projects.
- Maintain competence in clinical practice using continuing education opportunities,

Qualifications
- Graduate of an accredited Nurse Practitioner program; AND
- Received passing score on national certifying exam; AND
- Ability to be licensed in the State of IL as a Nurse Practitioner or Current Illinois Certified Nurse Practitioner license.
