Position Summary

The Community Care Advocate provides non-medical personal support and companionship to Sisters and residents in their home, helping create a welcoming, respectful, and dignified environment that reflects the mission and values of the Sisters of Mercy.

Using a holistic, person-centered approach, the Community Care Advocate supports individuals with varying levels of need during times of health, illness, transition, and recuperation. This position promotes comfort, independence, quality of life, and emotional well-being while working collaboratively with nursing staff and other interdisciplinary team members.

Key Responsibilities

  • Provide non-medical personal support and companionship to Sisters and residents in accordance with established support plans.
  • Foster a calm, respectful, and nurturing home environment that promotes dignity, comfort, independence, and emotional well-being.
  • Conduct routine rounding and observations, respond to nurse-call notifications, and promptly communicate changes in a resident’s condition or behavior to the appropriate personnel.
  • Provide reassurance and compassionate support during periods of anxiety, illness, transition, recuperation, and end-of-life.
  • Encourage and support participation in religious programming and individual or group activities that promote connection, community, and meaningful engagement.
  • Assist with meals and hydration while supporting the individual needs and preferences of Sisters and residents.
  • Perform light housekeeping, laundry, equipment cleaning, and other environmental support tasks.
  • Maintain a safe and welcoming home environment in accordance with established infection-control and safety standards.
  • Complete required documentation and basic administrative tasks accurately, promptly, and confidentially.
  • Provide transportation to scheduled appointments, activities, and outings.
  • Collaborate with nursing staff and interdisciplinary team members to promote clear communication and continuity of care.
  • Serve as the responsible on-site staff member when supervisory personnel are unavailable.
  • Exercise sound judgment and respond compassionately and appropriately to routine, urgent, and emergency situations.
  • Follow organizational policies, procedures, safety requirements, and confidentiality standards.

Required Qualifications

  • At least two years of experience in gerontology, long-term care, personal support, or another relevant setting.
  • Ability to provide respectful, compassionate, and person-centered support.
  • Ability to work independently while collaborating effectively as part of an interdisciplinary team.
  • Strong communication skills, including the ability to interact respectfully with Sisters, residents, co-workers, families, and visitors.
  • Ability to communicate effectively with individuals experiencing dementia or other cognitive changes.
  • Ability to understand and follow verbal and written instructions.
  • Ability to prioritize responsibilities, adapt to changing circumstances, and respond appropriately during urgent or emergency situations.
  • Demonstrated professionalism, dependability, sound judgment, and emotional resilience.
  • Ability to maintain confidentiality and respect the privacy and dignity of Sisters and residents.
  • Flexibility to work alternate shifts, weekends, and holidays as needed.
  • Valid driver’s license with a clean driving record.
  • Ability to perform the physical requirements of the position, with or without reasonable accommodation, including lifting, pushing, or pulling up to 25 pounds and sitting or standing for extended periods.

Preferred Qualifications

  • Previous leadership or mentoring experience, with the ability to motivate and guide others.
  • Experience working in healthcare, long-term care, senior living, faith-based, nonprofit, or mission-driven organizations.
  • Familiarity with Microsoft Office and general office technology.
  • Understanding and appreciation of the traditions, culture, and mission of the Sisters of Mercy and the Catholic Church—or a willingness to learn.
