Siouxland Community Health Center has an opening for:
Community Health Worker
1 – Full-time opening
Succesful candidate must have solid experience & advanced knowledge of:
- High School diploma or GED.
- Advanced computer and data entry skills
- Valid driver’s license
- Basic Life Support Certification (can be obtained after hire at SCHC)
- Mandatory Reporter Certification for Child & Dependent Adult (can be obtained after hire at SCHC)
- Complete Professional Skills Training for Community Health Workers within a year of hire.
- Complete health coach training (Fusion) and pass motivation proficiency within two years of hire.
Successful candidate must be able to perform primary functions of position:
- Assist patients in navigating health system at the Health Center, County, or State level.
- Access and communicate barriers in navigation to superiors.
- Provide basic education on need for screening tests, self- management of chronic diseases/wellness.
- Contact patients under supervision of clinicians to assess the need for additional services, compliance with treatment programs/plans.
- Travel to patients’ homes to assess barriers and help coordinate the reduction of those barriers.
- Assist quality staff with data projects and assist marketing staff with community events.
- These functions are not all inclusive
