Sinnissippi Centers (operating as a Care Coordination & Support Organization--CCSO) is hiring a new Care Coordinator to establish access to high-quality home and community-based services for children with significant behavioral health challenges. This System of Care improvement initiative (titled Pathway to Success) will focus on supporting children's success at home, in school, and in their communities in the Whiteside, Lee, Bureau, and Henry county areas. This position is home based and would focus on Whiteside County.

Job Duties
- Complete assessments for assigned clients to determine their level of need
- Develop care plans to reduce areas of health risk
- Assist assigned clients with accessing services to improve health, including healthcare, housing, education, nutrition, etc.
- Participate in multi-disciplinary staffings for assigned clients
- Provide support during episodes of a behavioral health crisis
- Meet with families and other formal and informal supports in the child's life to coordinate care (many of these meetings will be in person)
- Complete documentation of contacts and other information in required electronic systems

Requirements
- Must meet Illinois requirements of a Mental Health Professional, which can be met with a Bachelor's Degree in an eligible human service degree field.
- 1-2 years of experience coordinating care for high-risk populations.
- Ability to travel to client homes, Sinnissippi offices, and community sites.
- Ability to work independently.
- Proficiency with technology in a treatment setting.
- Ability to engage clients to meet treatment goals.
- The position can be office-based or home-based if an appropriate confidential home space is available.
Salary : $51,405 - $59,758
