Overview
We are seeking a compassionate and highly skilled Hospice Registered Nurse to join our dedicated palliative care team. The ideal candidate will possess extensive experience in various clinical settings, including acute care, critical care, and long-term care, with a strong focus on providing holistic end-of-life support. This role requires proficiency in EMR/EHR systems such as Epic, Cerner, and Athenahealth, along with a comprehensive understanding of medical documentation, medication administration, and patient assessment. The Hospice RN will deliver specialized care tailored to patients' physical, emotional, and spiritual needs while adhering to HIPAA regulations and quality standards such as NCQA. Experience in diverse areas such as dementia care, Alzheimer's care, trauma medicine, and infectious disease management is highly valued.

Duties

  • Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs.
  • Develop and implement individualized care plans aligned with hospice standards and patient preferences.
  • Administer medications including injections, IV infusions, and botulinum toxin treatments with precision and adherence to aseptic techniques.
  • Manage complex symptom control such as acute pain management, airway management, tube feeding, ostomy care, and ventilator support.
  • Provide emotional support and education to patients and families about disease progression, medication use, and end-of-life planning.
  • Coordinate with interdisciplinary teams for discharge planning, case management, and post-acute care services.
  • Document all patient interactions accurately within EMR/EHR systems like eClinicalWorks or Athenahealth while maintaining HIPAA compliance.
  • Participate in infection control practices and maintain sterile processing standards during procedures like phlebotomy or laboratory specimen collection.
  • Assist with specialized procedures such as spinal taps or gastrostomy placements when applicable.
  • Educate patients on health coaching topics including geriatrics, behavioral health issues, nutrition, and working with people with disabilities or developmental disabilities.

Experience

  • Proven clinical experience in hospice & palliative medicine with a minimum of 2 years in settings such as hospital medicine, ICU (Level I & II Trauma Centers), emergency room environments, or home & community care.
  • Demonstrated expertise in medical-surgical nursing, critical care transport, post-acute care, or outpatient clinics.
  • Familiarity with advanced medical procedures including cardiac catheterization, dialysis support, ostomy care, dermal fillers (nice-to-have), and sterile processing techniques.
  • Proficiency in using EMR/EHR systems such as Epic, Cerner, Athenahealth or similar platforms for accurate documentation.
  • Knowledge of ICD coding (ICD-9/10), CPT coding for billing purposes, and compliance with healthcare standards like NCQA standards is preferred.
  • Experience working with diverse patient populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia care), behavioral health patients, or those with infectious diseases is highly desirable.
