Job Summary
The Home Care Intake Coordinator Specialist is responsible for managing the intake process for new home care clients from initial referral through service start. This role serves as a primary point of contact for clients, families, referral sources, and internal staff, ensuring accurate information gathering, eligibility verification, timely scheduling, and exceptional customer service. The Intake Coordinator plays a critical role in client satisfaction, compliance, and operational efficiency.
Key Responsibilities
Intake & Referral Management
- Receive, review, and process incoming referrals from hospitals, physicians, case managers, families, and other sources
- Conduct initial client and family interviews to gather demographic, clinical, and service-related information
- Explain home care services, policies, and expectations to clients and families
Eligibility & Documentation
- Verify insurance coverage, authorizations, and eligibility requirements
- Ensure all intake documentation is complete, accurate, and compliant with state, federal, and payer regulations
- Maintain accurate records in the agency’s EMR or scheduling system
Coordination & Scheduling
- Coordinate with clinical staff, caregivers, and schedulers to initiate services in a timely manner
- Communicate client needs, preferences, and special requirements to the care team
- Monitor intake timelines to ensure prompt start-of-care
Customer Service & Communication
- Serve as a liaison between clients, families, referral sources, and internal departments
- Provide professional, compassionate, and responsive customer service
- Address intake-related questions or concerns and escalate issues as appropriate
Compliance & Quality
- Adhere to HIPAA, confidentiality standards, and agency policies
- Support quality assurance initiatives and intake performance metrics
- Participate in audits, reporting, and process improvement efforts
Qualifications
Required
- High school diploma or equivalent
- Minimum [1–3] years of experience in home care, healthcare intake, admissions, or a related healthcare administrative role
- Knowledge of home care services, insurance verification, and referral processes
- Strong organizational, communication, and multitasking skills
- Proficiency with EMR systems, scheduling software, and Microsoft Office
Preferred
- Associate’s or Bachelor’s degree in healthcare administration or related field
- Experience with Medicare, Medicaid, and managed care plans
- Bilingual abilities (preferred but not required)
