Job Summary

The Home Care Intake Coordinator Specialist is responsible for managing the intake process for new home care clients from initial referral through service start. This role serves as a primary point of contact for clients, families, referral sources, and internal staff, ensuring accurate information gathering, eligibility verification, timely scheduling, and exceptional customer service. The Intake Coordinator plays a critical role in client satisfaction, compliance, and operational efficiency.

Key Responsibilities

Intake & Referral Management

  • Receive, review, and process incoming referrals from hospitals, physicians, case managers, families, and other sources
  • Conduct initial client and family interviews to gather demographic, clinical, and service-related information
  • Explain home care services, policies, and expectations to clients and families

Eligibility & Documentation

  • Verify insurance coverage, authorizations, and eligibility requirements
  • Ensure all intake documentation is complete, accurate, and compliant with state, federal, and payer regulations
  • Maintain accurate records in the agency’s EMR or scheduling system

Coordination & Scheduling

  • Coordinate with clinical staff, caregivers, and schedulers to initiate services in a timely manner
  • Communicate client needs, preferences, and special requirements to the care team
  • Monitor intake timelines to ensure prompt start-of-care

Customer Service & Communication

  • Serve as a liaison between clients, families, referral sources, and internal departments
  • Provide professional, compassionate, and responsive customer service
  • Address intake-related questions or concerns and escalate issues as appropriate

Compliance & Quality

  • Adhere to HIPAA, confidentiality standards, and agency policies
  • Support quality assurance initiatives and intake performance metrics
  • Participate in audits, reporting, and process improvement efforts

Qualifications

Required

  • High school diploma or equivalent
  • Minimum [1–3] years of experience in home care, healthcare intake, admissions, or a related healthcare administrative role
  • Knowledge of home care services, insurance verification, and referral processes
  • Strong organizational, communication, and multitasking skills
  • Proficiency with EMR systems, scheduling software, and Microsoft Office

Preferred

  • Associate’s or Bachelor’s degree in healthcare administration or related field
  • Experience with Medicare, Medicaid, and managed care plans
  • Bilingual abilities (preferred but not required)

Job Type: Full-time

Pay: $47,000.00 - $57,000.00 per year
