Position Overview:
We are seeking a compassionate and reliable Part-Time Nurse (RN) to support our home care team. This role focuses on conducting Start of Care (SOC) visits, ongoing supervisory visits, and ensuring our clients—especially veterans—receive high-quality, compliant care.
This is an ideal opportunity for a nurse who values flexibility, autonomy, and meaningful patient relationships.
Key Responsibilities:
- Complete Start of Care (SOC) visits within required timeframes (typically within 72 hours of authorization)
- Conduct supervisory and follow-up visits in accordance with care plans and program requirements
- Develop and update individualized care plans
- Ensure compliance with VA and company documentation standards
- Communicate regularly with the care team regarding client needs, concerns, or changes in condition
- Provide education and guidance to caregivers to ensure quality care delivery
- Identify and report safety concerns, changes in condition, or additional service needs
Qualifications:
- Active RN license in the state of New Mexico
- Minimum 1 year of nursing experience (home care experience preferred)
- Strong clinical assessment and documentation skills
- Excellent communication and organizational abilities
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license
