We are seeking a strong, mission-driven leader to serve as a Care Coordinator, overseeing both a caseload of veterans and a team of case managers.

What You’ll Do
- Manage a caseload of veterans
- Ensure documentation and compliance standards are met
- Conduct field visits and maintain strong relationships with veterans and families
- Support onboarding and training of caregivers
- Coordinate care services
- Maintain strong relationships with VA and referral partners
- Promote accountability, professionalism, and high performance
This role blends leadership, field engagement, and operational oversight.
What We’re Looking For
- Bachelor’s Degree preferred
- Experience in case management, healthcare, or senior care required
- Leadership or supervisory experience preferred
- Strong organizational and communication skills
- Passion for serving veterans
