We are seeking a compassionate, organized, and customer service-driven Care Coordinator who thrives in a fast-paced environment and enjoys solving problems while supporting both caregivers and the veterans they serve.

Responsibilities
- Coordinate schedules for caregivers and clients while ensuring continuity of care.
- Match caregivers to clients based on availability, skills, and client preferences.
- Respond quickly to call-offs and last-minute scheduling changes.
- Communicate with caregivers, clients, family members, and office staff in a professional and compassionate manner.
- Visit clients in their homes as needed to provide support and maintain strong relationships.
- Assist with client care when necessary.
- Maintain accurate scheduling records and employee/client files.
- Ensure company policies and procedures are communicated and followed.
- Collaborate closely with Client Care, Nursing, Human Resources, and other office departments.
- Monitor open shifts and proactively work to prevent interruptions in client care.
- Help create an exceptional experience for both our veterans and our caregivers.
Qualifications
- 1-2 years of scheduling, care coordination, home care, healthcare, or customer service experience preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and time management abilities.
- Ability to prioritize multiple tasks in a fast-paced environment.
- Detail-oriented with excellent problem-solving skills.
- Comfortable using computers and learning scheduling software.
- Ability to remain calm under pressure and adapt to changing priorities.
- Positive attitude with a strong sense of accountability.
- Valid driver's license and reliable transportation.
