Key responsibilities include:
- Coordinating follow-up care after hospital or facility discharge
- Engaging patients regarding preventive screenings, chronic condition management, and recommended follow-up care
- Serving as a liaison between patients, providers, insurance resources, and community services
- Documenting patient interactions accurately and timely
- Supporting quality improvement and population health initiatives
- Collaborating with physicians, hospitals, home health agencies, and other healthcare partners
- High school diploma or equivalent required
- Associate degree or health-related education preferred
- 3 years of experience in a healthcare, physician practice, outpatient, or hospital setting preferred
- Strong communication and organizational skills
- Comfortable working independently in a remote environment
- Proficiency with Microsoft Office applications
- Ability to manage deadlines while maintaining professionalism and confidentiality
- Ability to travel to clinics within the North Central Florida area (limited)
