Social Worker LCSW or LMSW - Case Management

SilverSource is seeking an experienced Social Worker to join our team. This position reports to the Chief Program Officer and requires a minimum of 6 years of experience in a leadership role. Experience with older adults is preferred.
What you will do: Responsibilities
· Consult with clients and their families in the office, home or senior site as appropriate to provide assistance and referrals.
- Administer a comprehensive intake evaluation to assess clients’ physical and mental wellness, critical needs, preferences and abilities and vulnerabilities,
- Develop plans of action to improve the client’s well-being, identify solutions, and work on goals with client.
- Screen all clients for eligibility to Federal, State and local programs.
- Provide services such as financial assistance, application support, budgeting help, solution-focused short term counseling, and/or referrals to outside agencies/providers.
- Liaise with outside agencies such as Protective Services for the Elderly, City agents, Dept. of Mental Health and others as needed for collaboration on client care.
- Develop and maintain positive collaborative relationships with community agencies and professionals to enhance resources to meet the needs of clients
- Actively participate in interdisciplinary planning team meetings, supervision and other staff development activities
- Utilize the agency’s electronic record system to ensure proper and timely completion of documentation, program statistics, and outcome data
- Other related activities as assigned
- Periodic follow up with clients to ensure they are safe, stable and in good physical and mental health
- Participate in weekly case management and biweekly staff meetings.
- Attend agency functions as appropriate
Requirements
- Master’s in social work or LCSW preferred
- 6 years’ experience working with seniors or vulnerable populations preferred
- Must have a positive person-centered demeanor
- Must be proficient in MS Office (Outlook, Word and Excel)
- Must have the ability to learn new software and enter data accurately
- Must be detail-oriented and have the ability to multi-task
- Must be capable of handling a rolling caseload of up to 35 clients

Work Environment
This job operates in a collegial, professional office environment. This role routinely uses standard office equipment such as computers, phones, fax, photocopiers, scanners and associated software applications.
Position Type/Expected Hours of Work
This is a full-time position, and hours of work and days are Monday through Friday, 9:00 AM to 5:30 PM with ½ hour for lunch.
Travel
Travel is generally limited to doing in-home visits, representing the agency at other entities in Stamford, and attending agency functions.
