The Opportunity:
Silvergate Fallbrook, part of AmeriCare Health & Retirement, is seeking a compassionate and skilled Part-Time LVN to support our award‑winning senior living community. If you’re a nurse who values teamwork, resident-centered care, and a supportive work culture, we’d love to meet you.
What You’ll Do:
- Provide clinical oversight and clear direction to caregiving staff
- Assess residents, document changes, and communicate with physicians and families
- Perform diabetic care, medication tasks, and follow physician orders
- Coordinate medical appointments and support proactive health management
- Partner with Home Health/Hospice teams to ensure comfort and continuity of care
- Train, coach, and support caregiving staff
- Uphold infection control and regulatory standards (Title 22)
Minimum Qualifications
- Current California LVN license
- Current CPR and First Aid certification
- 2 years nursing experience (assisted living, memory care, or SNF preferred)
- Strong communication, leadership, and teamwork skills
- Knowledge of gerontology, dementia care, and resident rights
- Ability to remain calm and effective in emergencies
