Field RN Case Manager – Home Care

Silver Touch Home Care is looking for a competent RN Case Manager PRN to care for our clients in a professional and compassionate manner. It is also the requirements of the RN to provide the training to current caregivers. The Field RN Case Manager will visit approx. 10 clients per month and complete supervisory visits, skills validations/trainings and medication lists for our clients who have Health Maintenance Activities under the In Home Services Support program with Medicaid. Each visit will pay $50 per client and mileage reimbursement.

Job Requirements:

• RN License Required.
• Ability to work independently.
• Ability to travel to multiple clients in various cities across Boulder, Weld, Denver, Arapahoe and other Counties.
• Excellent documentation skills with a solid grasp of computer usage.
• Provide effective communication to patient/family, team members, and other health care professionals.

Silver Touch Home Care is a class B non medical home care agency and our office is based out of Aurora, CO. We provide care statewide in Colorado, including Adams, Arapahoe, Broomfield, Clear Creek, Douglas, El Paso, Pueblo, Weld, Boulder, Jefferson, Larimer, and Gilpin counties.

Pay: $50.00 per hour
