What are the responsibilities and job description for the Family Nurse Practitioner Part-time consistent days position at Silver Hill Medical Clinic?
Silver Hill Medical is a Privately Owned Nurse Practitioner Clinic that is looking for an addition to our growing practice. The Family Nurse Practitioner shall provide primary care and walk-in care to Silver Hill Medical Clinic’s patients that are consistent with established treatment protocols; and as directed by the supervising physician.
ESSENTIAL DUTIES
1. The practitioner shall see the patient at the earliest opportunity:
· Take care of the patient's immediate needs.
· Undertake medical evaluation, including history and physical examination, laboratory, X-ray, referrals, or other diagnostic studies.
· Evaluate the patient and develop a problem list.
· Develop treatment objectives and plans.
· Initiate treatment orders and procedures.
· Work closely with other staff members in coordinating and obtaining needed services for patients
· Educate and counsel patients and their family to promote good health and disease prevention
· Use and disclose patients’ protected health information only in accordance with the Health Insurance Portability and Accountability Act (HIPPA) standards.
· Practice within the confines of established treatment protocols, and as directed by the supervising physician.
· Consult with the physician supervisor whenever any question arises as to diagnostic evaluation or medical treatment of any patient
2. The practitioner will make every reasonable effort to follow through personally with the patients she/he evaluated initially.
3. Patients will be asked to return for further services only as often as necessary and by appointment whenever possible
4. Recording of Patient’s Progress, Procedures, and Other Matters:
a. A visit will be recorded, and the chart note accepted in the EHR within seven days of the encounter.
b. Notes should:
i. Reflect change in a patient’s condition and other significant information.
ii. Include documentation of pertinent history, review of systems, and physical exam
iii. Indicate studies performed and that results are reviewed and discussed with the patient.
iv. Reflect provider’s thought processes as to the patient’s problems, diagnosis and treatment
v. Document the need for continuing services and the appropriateness of diagnostic evaluations and treatment plans.
vi. Include referrals to specialty clinic services as determined to be medically necessary.
vii. Specify medications prescribed by the provider
viii. Reflect any pertinent attitudes or behavior on the part of the patient or family towards any aspect of care.
ix. Include notation of the need for routine and preventive services such as screening tests, periodic laboratory examinations related to chronic disease care, and immunizations
c. Results (including but not limited to labs, radiology results, consult notes, outside records) received by the provider should be attended to within 7 days, with acutely abnormal results attended to more urgently.
d. On-Call notes should be entered into the patient’s chart within 3 days.
e. Use, protects, and discloses Silver Hill Medical Clinic’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
5. Refer patients for specialty clinic services if medically necessary.
6. Integrate recommendations made by subspecialists into the patient treatment plan.
7. Inform the patient of the general nature of his/her medical problem, the general prognosis and the nature and purpose of the proposed treatment and procedures; provide appropriate instructions in self- care to the patient and document in the patient's medical records.
8. Refer for a second opinion if requested by the patient’s family, and deemed medically reasonable
9. Develop and maintain a current chronic problem list
10. Promote a cohesive, cooperative working relationship with fellow providers which may include helping with excess patient loads and staying after the scheduled working hours.
11. Completes continuing education as required by state licensing standards.
12. Develop and maintain professional working relationships with patients.
13. Participate in Silver Hill Medical Clinic’s after-hours call system only when arranged with supervisor.
14. Ensure Compliance with Productivity and Documentation Standards:
a. Ensure productivity standards are met. Goal is for provider to see patients based upon walk-in and scheduled needs of the patient. To attempt to make every effort to see patient’s same day if possible. Notify supervisor is patient load is too great to complete in a safe manner
b. Completes documentation and coding as required for patient care.
Secondary Duties
Perform related duties as required, including assuming full or partial responsibility for major projects and initiatives designed to support the success and mission of Silver Hill Medical Clinic
Supervisory Responsibilities
This position has no formal supervisory duties. However, the Nurse Practitioner is expected to provide thoughtful leadership relative to human resources functions throughout the organization. This may include providing clinical supervisory over staff, interns, students, and volunteers as assigned and when alone at the clinic.

POSITION REQUIREMENTS
These specifications are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this job. Individual abilities may result in some deviation from these guidelines. To perform effectively in this position, the incumbent must have:
1. License to practice as a family nurse practitioner in the State of Tennessee.
2. Board certified or board eligible in field of specialty.
3. Prefer at least 1 years clinical experience as a nurse practitioner. Knowledge and experience in walk-in primary care preferred.
4. Effective communication and interpersonal skills with children and families.
5. Timely completion of continuing education necessary to meet state licensure.
6. Work Friday and every other Thursday with a Minimum of 1 Saturday per month 9am-2pm
